How to Find Us

Burlington Area Family Practice Center is conveniently located on the corner of West Agency Road and Gear Avenue South in West Burlington, IA on the Great River Medical Center campus, near Southeastern Community College and Westland Mall. 

If you're arriving from East or West on US Highway 34, take the Gear Avenue exit in West Burlington and travel South to Agency Road. Staying on Gear Avenue, go through the Agency Road intersection then take the first right turn into our parking lot.

If you're arriving from North or South on US Highway 61, exit West on US Highway 34, take the Gear Avenue exit in West Burlington and travel South to Agency Road. Staying on Gear Avenue, go through the Agency Road intersection then take the first right turn into our parking lot.
